Please read along as your facilitator reads this aloud: You are about to take a survey that is very important because the information you provide will help us understand the knowledge, attitudes, and behaviors of students like you. So please answer the questions carefully and honestly. Some of the questions in the survey are very personal because they ask about topics related to sex.
We promise that your answers to the questions will always be absolutely confidential. This means that no one will ever know that it was you who answered the questions. Your answers will be grouped with other students’ answers.
You will not put your name on the survey – only a unique code that you will develop as one of the first questions to the survey. When developing the unique code, please use the name that is on your birth certificate; do not use a nickname or other abbreviation because you need to create the same unique code for each survey – that’s how we’ll be able to track your answers over time.
Some of the questions ask about things that happened in the past 3 months and some questions ask about things that may happen in the next 3 months. By the past 3 months, we mean what happened between June 2014 and now. When we are asking about the next 3 months, we mean between now and November 2014.
When you finish the survey, you will be directed to another website. Please explore the website and do not disturb the other students.

Now, please begin the survey. It begins by confirming your participation and asking you to create your confidential ID and a few other basic questions. Then, the questions will move on to more questions about your beliefs and experiences. If there is something you do not understand in the survey, just raise your hand and a facilitator will come to help you.
